What does it mean when my car shifts hard?

I have a 1985 Lincoln Towncar. Let me first say that last weekend the problem started, I jacked the car up and removed the transmission pan on the underbelly and cleaned what I could clean hoping that would solve the problem but it did not . In order for me to drive back and forth to work I must tollerate the hard shifts. Please tell me what I can do about this?
