Rotator cuff tear or frayed?

I have an appt. to see an arthroscopist in 4 more days and the pain is just too hard to bear. I am a veteran and can only go thru the VA for medical care, otherwise it's out of pocket. Motrin , heat, ice. , nothing is helping the pain at all.i am limited with pain meds from previous opiate abuse and I just need help from someone who cares enough to give me some suggestions to releave this pain!
